Please submit this form to request an exemption from the SCRA Evaluations process if your player will be absent from all or some of the evaluation sessions due to injury, illness or family emergency.
If granted, the exempted player will be placed on a team (or within the next round of evaluation sessions if applicable) by the SCRA Executive.
Any personal health information submitted will be kept confidential and will only be used for this express purpose.
