Please submit this form to request an exemption from the SCRA Evaluations process if your player will be absent from all or some of the evaluation sessions due to injury, illness or family emergency. (This includes absence due to COVID-19 illness or isolation.) Once this form is received, we may follow up with you for further information and/or documentation such as a doctor’s note.
If granted, the exempted player will be placed on a team (or within the next round of evaluation sessions if applicable) by the SCRA Executive. Any personal health information submitted will be kept confidential and will only be used for this express purpose.
